    • This form must be filled out completely and submitted with photos of each claim area, description(s) of the claim areas before the claim will be evaluated for approval.
    • Please allow 2-4 weeks for all claims to be evaluated. You may be contacted by a representative during this time for additional information.
    • Product/material replacements will be made upon product availability. Product may not be available at the time of claim processing. If not available, a replacement order will be back-ordered and then repair scheduled upon availability.
    • If the exact product/material cannot be procured, a like kind replacement will be used.
    • Substitutions of products, colors, styles, sizes, etc. Will not be accepted.
